장내 균총이상이란

장의 구조와 역할
장은 십이지장, 공장(空腸), 회장(回腸)으로 구성된 약 6~7m 길이의 소장과 맹장, 충수, 결장, 직장으로 구성된 1.5m 길이의 대장으로 구성되어 있다. 지금까지 이 7.5m 길이의 장의 중요성에 대해서는 소화기관으로써의 기능 이외에는 크게 주목 받지 못하였으나 우리 몸에 들어오는 병원균의 90%가 소화기를 통해 침입하며 우리 몸 전체 면역시스템의 70~80%가 장관에 존재할 정도로 중요하다. 또한 장 점막 표면은 300~400㎡나 되고 우리 몸과 장내 미생물 사이에 가장 넓은 연결 부위인데 미생물 생태계로 구성된 장벽, 점액질로 덮여 있는 상피세포의 물리적 장벽, 분비 IgA를 포함하는 면역 장벽의 3중 구조로 우리 몸을 보호하고 있다(1, 2). 그리고 장에는 척수만큼 많은 신경조직 뉴론을 갖고 있고 세라토닌의 95%가 장관에서 만들어져 장 신경 시스템은 제2의 뇌라고 불리고 있다. 이러한 장의 구조적 기능의 중요성과 더불어 최근 장에서 서식하고 있는 미생물의 기능적 중요성이 대두되고 있는데, 그 동안 장내 미생물과 장의 구조와는 연관성이 없는 것으로 생각되었으나 충수(appendix)가 장내 미생물과 연관된 기관임이 최근 밝혀진 바 있다. 즉, 대장의 막창자(맹장)에서 0.5~1㎝ 폭에 5~10㎝ 길이로 연충 모양의 파우치 형태로 나와 있는 충수의 기능에 대해서 오랫동안 논쟁의 대상이었는데, 일종의 장내 공생 미생물의 안전 가옥 역할을 하여 장관 내의 내용물이 제거되는 상황이 발생되어 병원균에 노출될 경우 공생미생물의 생육을 돕고 대장에 공생 미생물을 재 접종하는 역할을 하는 것으로 밝혀지고 있다(3).
장내 미생물의 구성과 그 중요성
장내 미생물은 700~1,000여 종의 미생물로 구성되어 있으며 인체 세포수의 10배 정도인 100조 개로 구성되어 있다. 무게는 1.5~2kg 정도이며 분변의 60%를 차지하는 것으로 알려져 있다. 이들은 인체 세포보다 크기가 훨씬 작기 때문에 체중에서 차지하는 비중은 1∼2%에 지나지 않지만 인체에서 배출되는 노폐물의 50% 이상이 이들 미생물이 만든 것이다. 이 장내 미생물은 고유한 구조와 구성으로 미생물 생태계를 구성하고 있는데 서로 대사적으로 연관된 미생물의 단단한 결합체이며 숙주레벨에서 자연선택 되는 것으로 밝혀 졌다. 이는 장내미생물은 숙주와 함께 오랜 세월 같이 진화해 왔으며 섭취 음식과 숙주의 계통 분류에 따라 미생물 균총의 다양성이 달라진다고 한다(4, 5). 최근 연구에 의하면 육식성, 잡식성, 초식성 포유류 순으로 장내 미생물 다양성이 증가하는데 현대적 생활을 하는 인간은 전형적인 잡식성 영장류의 장내 균총으로 분석되고 있다(6). 장내 미생물은 숙주의 건강에 직접적으로 영향을 주는데 장의 면역 시스템 자극, 장 상피세포 증식 조절, 비타민 K 및 B 계열의 필수 아미노산 합성, 병원균에 대한 저항성 증대 등의 작용을 하며 미생물 균총의 불균형이 많은 만성 질병의 원인이 되는 것으로 알려져 있다(7-11). 그 외에 심장 크기에 영향을 주는 등 미생물 균총과 인체 생리학적 측면과의 연관성도 밝혀지고 있으며 장내 미생물에 의해 직 간접적으로 숙주 유전자 발현을 조절하여 신경활성물질의 생산에 영향을 줄 수 있어 신경발달이나 정실 질환에도 관여 가능성이 높은 것으로 밝혀지고 있다(12)
물질대사 측면에서 보면 장내 미생물 균총이 간보다 대사기능이 100배 이상 큰 것으로 알려져 있다. 즉, 마이크로비옴(전체 장내 미생물 유전체) 측면에서 보면 장내 미생물 균총이 500~1,000 종의 미생물로 추정할 경우 200~400만개의 유전자가 장내에 존재하는 것으로 인체 유전자 종류의 70~140배에 해당한다. 따라서 이들 유전자 산물인 다양한 대사관련 효소들이 소화하기 어려운 영양분을 분해하여 숙주에 유익한 물질로 전환하며 식물화학물질등을 대사하여 생리활성 물질로 바꾸어 주거나 환경독성물질을 무독화 시키는 역할도 수행한다(13-16). 또한 미생물 균총은 복용한 약물의 약물 대사와 생체 이용률과 관련되어 있으며, 식품의 영양과 에너지적 가치는 절대적인 것이 아니라 부분적으로는 개인별 장내 미생물 균총의 대사 능력에 영향을 받는다는 것으로 보고되고 있다(12). 이러한 장내 미생물의 인체 장기와 차별되는 다양한 기능의 중요성으로 인해 제3의 장기(organ)로 불리고 있으며, 장의 건강은 장내 미생물과 떼어서는 설명할 수 없을 정도로 장내 미생물 균총의 항상성이 중요하다는 것이 밝혀지고 있다.
장내 균총 이상과 건강에 주는 영향
성인의 장에는 셀 수 없이 많은 수의 미생물이 존재하는데 체세포 수의 10배에 해당하는 100조개 정도가 있다. 크게 세균, 고세균, 진핵생물로 구성되어 있으며 이러한 미생물 군집은 숙주의 장기 안에 있는 미생물 장기 (microbial organ)으로 볼 수 있으며 인체 유전체 보다 100배 이상의 유전체를 갖고 있다. 앞서 설명한 데로 일단 미생물 균총이 형성되면 많은 중요한 기능을 하고, 건강한 상태의 인체는 장내 미생물 균총과 우리 몸 사이에 항상성(恒常性)이 유지된다. 그러나 장내 미생물 구성이 바뀌는 균총 이상(dysbiosis)라는 항상성이 깨지는 상태가 되면 소화기 질환, 대사관련 질환, 암, 자가면역질환 등 여러 가지 질병이 생기게 된다. (17-31). 특히 그 동안 관련성이 없어 보였던 신경과 관련된 질환과 행동 장애도 장관 내 신경조직이 뇌와 연결되어 있기 때문에 장내 미생물 균총 이상과 관련이 있는 것이 밝혀지고 있다(32, 33). 또한 최근 연구에 의하면 장내 균총이상이 되면 병원균 같은 유해균이 과증식하는 조건으로 바뀌고 이러한 장내 생태계에서 구성 미생물들간의 수평적 유전자 전이가 촉진되어 항생제 내성이나 병원성 관련 유전자들이 병원성 미생물의 진화를 촉진하게 되는 것으로 알려졌다(34).
